

Free Through Recovery is a community based behavioral health program designed to increase recovery support services to individuals involved with the criminal justice system who have behavioral health, housing, employment, recovery programs needs.

Individual Eligibility
To meet the criteria for Community Connect an individual must meet the following criteria:
Care Coordination
A care coordinator assists with short and long-term goals, barriers, and provides a source of connection and support.
Peer Support
A peer support specialist is an individual with similar lived experience that offers hope, support and advocacy.
Recovery Services
Access to services or programs to assist an individual with meeting their needs and goals.

The North Dakota Medicaid 1915(i) State Plan Amendment allows North Dakota Medicaid to pay for home and community -based support for individuals with behavioral health conditions.
Eligible individuals will meet each of the following criteria:
